Need to send us a file?

If you are already a client of National Disability Alliance, you can use our questionnaire to send us documentation related to your case.

The questionnaire will request you to upload one file at a time and provide an explanation for each file.

You may provide your address so that our team can sort your files into the proper client file. However, this step is not required.

Please note that if you do not already have a client relationship with us, this communication will not establish one.
